AGREEMENT

Tournament teams are placed on a first come basis, in the order they are received.

If a division is not formed, your deposit will be returned to you.

Team Roster/Waiver forms need to be signed by each players parent/legal guardian (if under the age of 18) and turned in to a Genesee Fieldhouse representative by the first game.

If paper-work is not received you may forfeit each game until this information is turned in.

I understand that I am responsible for turning in all Roster/Waiver forms, understanding the tournament rules as set forth for this event, and any payments that are returned for insufficient funds are subject to a $25.00 ISF Fee.
